Constant Change focuses on children and adolescents who are at risk for removal from the home due to behavioral, mental health, or substance abuse problems. Additionally, children who are transitioning back home from an out-of-home placement such as foster care, detention, or other residential programs may be appropriate for services. Services are available to clients from the ages of 5 to 17 years old.
This service is for adults and children in their late teens who suffer from mental illness and lack basic living skills, need connections to community resources and require help in managing their psychiatric symptoms. Our staff helps create a specialized treatment plan with goals specific to the client’s needs, emphasis on managing psychiatric symptoms and establishing consistent medication management. This service improves a client’s well being by establishing a support network of safety, education and empowering the client to increase overall quality of life by managing and controlling symptoms. Our services include extensive collaboration with outside professionals and 24/7 crisis management.
Constant Change Enterprises, LLC (CCE) Outpatient MH Service provides structured treatment to serve high-risk populations using a Person-Centered approach by collaborating with family members, school personnel, physicians, mental health providers, and community service providers. Our approach is to assist individuals, couples, and families ages 4-65 who are having trouble with managing mental health and/or behavioral disorders.
Outpatient treatment modalities that we offer include individual therapy, family therapy, couples’ therapy, and group therapy. Some of the issues that we address are anxiety, depression, bipolar disorder, PTSD, ADHD, substance abuse, trauma, abuse and neglect, grief and loss, foster care and adoption, anger management difficulties, family/relationship conflicts, and behavioral difficulties.
Community Stabilization services are offered to individuals experiencing an acute crisis of a psychiatric nature that may jeopardize their current community living situation.
Constant Change Community Stabilization will serve individuals of all ages. These services are time-limited interventions provided typically but not solely in the residence of the individual who is at risk of being hospitalized.
